New or Lapsed/Re-applying Licensing Process

As of January 1, 2017, all new or lapsed/re-applying EMS provider license applicants must complete a Criminal Background Check.

The process for the background check is outlined below:

IMPORTANT NOTE: LiveScan fingerprints are only valid for 30 days
  • EMS Provider License Application Adobe Acrobat Reader Symbolfor New or Lapsed Applicants;

  • NH Approved Scope of Practice Modules at the appropriate provider level. This can be found can be found under the Continuing Education section at: https://www.nhfaemslearning.org

  • NH Protocol Program/Exam at the appropriate provider level. This can be found under the Continuing Education section at: https://www.nhfaemslearning.org

  • National Registry of Emergency Medical Technicians (NREMT) Certification

  • Initiate State Police Criminal Records Unit (CRU) criminal history record information (CHRI) background check either online or through paper form submission;
    • Electronically at: https://services.dos.nh.gov/chri/cpo/. This includes the application, scheduling your fingerprint appointment and paying the processing fee of $48.25. Be sure to select “Emergency Medical & Trauma Services” for the correct “Agency” when applying for the required EMS Provider background check (NOT "Medical Technician" which is a different license.)
      -OR-
    • Paper Form Submission: An EMS Provider applicant can continue to submit paper documents (inked fingerprint card, check (fee of $48.25) and CHRI form) to the Bureau of EMS for processing. Fingerprinting by "inked" or, if available, LiveScan electronic process can be completed by Local PD and the “tracking form” submitted.
    • Completed EMS Provider License Application for New or Lapsed Applicants;
    • Proof of National Registry of Emergency Medical Technicians (NREMT) Certification;
    • CHRI Component;
      • A note stating that you have submitted the CHRI online directly to the CRU
        -OR-
      • Completed CHRI Authorization form (a)
      • Fingerprint Card OR LiveScan receipt/tracking form (b)
      • Payment ($48.25) made out to "State of NH-Criminal Records Unit
