The following must be appropriately completed:

• EMS Provider License Application for Renewal/Upgrade/Change of Unit Applicants;
• NH Approved Scope of Practice Modules at the appropriate provider level. This can be found can be found under the Continuing Education section at: https://www.nhfaemslearning.org
  
• NH Protocol Program/Exam at the appropriate provider level. This can be found under the Continuing Education section at: https://www.nhfaemslearning.org

Choose either hard copy or online application process depending on how you would like to submit. It is no longer necessary to submit copies of the protocol exam, or Scope of Practice (SOP) completion certificates to the Bureau of EMS.
