Individuals name change requires proof from a governmental agency such as marriage/divorce decree, or similar document. Fax, e-mail, or mail to the Licensing Division. No fee is required and new license will not be sent.
Business Entity
- Name change without FEIN change: Send request by letter, fax or email from officer or DRLP. No fee is required and new license will not be sent. New name should be registered with New Hampshire Department of State Corporate Division.
- Changing FEIN: indicates a new or different legal entity. New application procedures apply.
Address Change (must be completed within 30 days of the change).
Effective January 1, 2012, any adjuster with a state to state residence change is no longer required to complete a new application and submit the amendment fee of $50. We do require the paper address change form and a $10 address change fee.
