Individuals name change requires proof from a governmental agency such as marriage/divorce decree, or similar document. Fax, e-mail, or mail to the Licensing Division. No fee is required and new license will not be sent.

Business Entity

• Name change without FEIN change: Send request by letter, fax or email from officer or DRLP. No fee is required and new license will not be sent. New name should be registered with New Hampshire Department of State Corporate Division.
• Changing FEIN: indicates a new or different legal entity. New application procedures apply.

Address Change (must be completed within 30 days of the change).

Effective January 1, 2012, any adjuster with a state to state residence change is no longer required to complete a new application and submit the amendment fee of $50. We do require the paper address change form and a $10 address change fee.

• If changing domicile/resident state, the ONLINE systems CANNOT be utilized. Address change form and fee is required for Producers and Adjusters.
• Change address electronically without a fee using the National Insurance Producer Registry (This function may be used by all Individual Producers, All NH Resident Adjusters, and Non-Resident Adjusters who have a home state license).

• Print Address Change Form and mail to Licensing Division

• Obtain Letter of certification online from the License Verification Look-Up Service
• Send written request via letter with $10 fee

Letter of Clearance (Proof of surrender of New Hampshire license)

• Submit written request via letter with $10 fee, submit your license, and tell us where you would like the letter mailed

Duplicate License

• Register for SBS Connect (free service).
• Subscribe to the Online Licensing Service (fee required).
• Send written request with $10 fee
