Individuals name change requires proof from a governmental agency such as driver’s license, marriage/divorce decree, or similar document. Fax, e-mail, or mail to the Licensing Division. No fee is required and new license will not be sent.

Business Entity name change without FEIN change requires new registration with New Hampshire Department of State Corporate Division and $50 amendment fee.

Address Change (must be completed within 30 days of the change).

• Change address electronically without a fee using the National Insurance Producer Registry (This function may be used by all Individual Producers, All NH Resident Adjusters, and Non-Resident Adjusters who have a home state license).
• Subscribe to the Online Licensing Service
• Download Address Change Form and transmit to Licensing Division

Letter of Certification (certifies current standing)

• Obtain Letter of certification online from the National Association of Insurance Commissioners
• Send written request via letter with $10 fee

Letter of Clearance (Proof of surrender of New Hampshire license)

• Submit written request via letter with $10 fee, submit your license, and tell us where you would like the letter mailed

Duplicate License

• Subscribe to the Online Licensing Service subscription service
• Send written request with $10 fee
