Variable Products Application for Licensed Life Insurance Companies

Application Form to issue Variable Products Contracts
Filing fee* Fee is the higher of New Hampshire's fee of $100 or company's state of domicile. Make check payable to "Treasurer, State of New Hampshire." *All fees are retaliatory and non-refundable.

Mail all items to the New Hampshire Insurance Department, Financial Regulation Division.

Pursuant to RSA 408:27-36, a life company licensed to do business in the State of New Hampshire must obtain an additional license for the issuance of Variable Products Contracts.
