Application for Pre-Screening of School-to-Work Business Partner

Welcome to the State of New Hampshire Department of Labor's Request for Pre-screening a School to Work Business Partner online application. We are in the process of redesigning this web site and migrating it to the State's NH.gov server. The online application form for School-to-Work is not yet available. There are presently two options for submitting a request for pre-approval of a School-to-Work Business Partner.

Option One: Download the Adobe PDF version of the form , print it, complete it manually and either fax or mail it in. See the fax and mailing address below.
