On May 21, 2019, at approximately 11:14 a.m., Trooper David Skelly of the New Hampshire State Police – Troop G arrived at an authorized State of New Hampshire Safety Inspection station located in the town of Wakefield, New Hampshire in order to follow up on reports of unsafe vehicles receiving official State of New Hampshire Safety Inspection stickers.

While Trooper Skelly was at the station, he observed that Edward W. Cushing, age 48, of Ossipee, NH, was in the process of conducting a State Safety Inspection on a motor vehicle. Trooper Skelly was able to observe noticeable safety violations on the vehicle which caused it to fail a State Safety Inspection. Even after the safety violations were brought to Cushing's attention by Trooper Skelly, Cushing proceeded to issue an official State of New Hampshire Safety Inspection sticker to the vehicle.

Cushing was arrested by Trooper Skelly and charged with Unauthorized Display of an Official Inspection Sticker. Cushing was released on Personnel Recognizance bail and ordered to appear in the 3rd Circuit – District Division – Ossipee Court on June 6, 2019.
