Motor Vehicle Crash, Route 16, Milton

On Friday, June 17, 2016, at approximately 2219 hours, troopers assigned to the New Hampshire State Police Troop A barracks responded to a report of a multi- vehicle accident on Route 16, in the area of Exit 17 in Milton, New Hampshire. Officers from the Milton Police Department responded as well, and they were able to close the roadway down in both directions at Exit 17. Upon arrival, troopers encountered multiple vehicles in various portions of the paved roadway to include the breakdown lane.

Preliminary investigation revealed that Unit #1, a 2004 Dodge Stratus, being operated by Sarah O'Halloran, age 31 of Sanbornville, New Hampshire, was driving southbound south of Exit 17, when she encountered a large, orange, lighted arrow board in the middle of the roadway. Due the ongoing construction in the area, the sign had been placed in that location to provide early warning of a lane shift in the roadway. For a reason that remains under investigation, Ms. O'Halloran failed to negotiate the lane shift and she drove into the arrow board, knocking it on its side and pushing it into oncoming northbound traffic. As traffic stopped on the northbound side due to the unexpected road obstruction, Unit #2, a 2015 Chevy Colorado, being operated by Thomas Clem, age 53 of Woonsocket, Rhode Island, had stopped approximately ten vehicles back. Unit #3, a 2004 Nissan Altima, being driven by Monica Nasser, age 30, of Cambridge, Massachusetts was also stopped in traffic directly behind Unit #2. Unit #4, a 2010 Honda Accord, being driven by Carletta Murphy, age 25, of Rochester, New Hampshire was unable to stop in time and drove into the back of Unit #3. Unit #3 was then pushed into Unit #2 causing minor rear end contact damage to Unit #2. As a result of the impact, Unit #3 sustained significant front and rear end damage. Unit #3 subsequently became fully engulfed in flames, and the vehicle was completely destroyed by the fire. Unit #4 sustained significant contact damage to the front end.

As a result of the crash Carletta Murphy was transported by Milton Fire-Rescue to the Frisbee Memorial Hospital in Rochester with minor injuries. There were no other injuries reported at the scene.

Due to the placement of the vehicles in the roadway, Route 16 was completely shut down at Exit 17 north and south for approximately two hours while the crash was investigated, the vehicles were removed from the roadway, and the construction sign boards and equipment were replaced.

Units #1, #3 and #4 were all removed from the scene by Crowell's Towing and Repair of Wakefield, New Hampshire.

The New Hampshire State Police were assisted at the scene by members of the Milton Police Department, the Milton Fire Department and the New Hampshire Department of Transportation.

This crash remains under investigation, and anyone with information pertaining to this incident is asked to contact Trooper John Forbes, New Hampshire State Police, Troop-A, at (603)223-8490 or via email at
