To obtain a Driving Record, also known as Driver History Record/Report, you must complete the NH Motor Vehicle Record Release FormAdobe Acrobat Reader Symbol(DSMV 505). Below, please see the different types of requests and specific instructions to assist you in obtaining a Driving Record:

Obtaining Your Own Driver Record Report

You must complete the following fields of the NH MV Record Release FormAdobe Acrobat Reader Symbol(DSMV 505).

  • Step 1 "What information are you requesting from the DMV?" - Select "Driver record" from the Driver Information heading.
  • Step 2 "Who are you?" - Select the first box indicating that you are the record holder or vehicle owner.
    • Whose information are you looking for? - Fill out the required information on the right with your information. Minimum information required to complete your request includes:
      • Name (full).
      • Date of Birth.
      • License/Non-Driver Identification Number.
  • Step 3 "Information of the person filling out this form" - You, as the requestor, are required to provide your full name, mailing address, and a contact telephone number.
  • Step 6 "Important!!! Please read the penalty clause below" - All requestors must read the penalty clause.
  • Step 7 "Signature is required" - You, as the requestor, must sign and date the form.
  • Step 8 "Submit your request" - Present the form with a $15 payment to the Colebrook, Concord, Dover, Epping, Gorham, Keene, Manchester, Milford, Nashua, Newport, North Haverhill, Salem, Tamworth or Twin Mountain DMV offices, or you may mail the completed form along with the $15 payment to:

Releasing of Your Driver Record Report to Another Individual or Entity

If you wish to release your record to a third party, you must complete the following fields of the NH MV Record Release FormAdobe Acrobat Reader Symbol(DSMV 505). Please note that these requests may only be processed at the Concord DMV office, as indicated below.

  • Step 1 "What information are you requesting from the DMV?" - Select "Driver record" from the Driver Information heading.
  • Step 2 "Who are you?" - Select the box indicating that you, the record holder, have approved this request.
    • Whose information are you looking for? - Fill out the required information on the right with your information. Minimum information required to complete your request includes:
      • Name (full).
      • Date of Birth.
      • License/Non-Driver Identification Number.
  • Step 3 "Information of the person filling out this form" - Please fill out the appropriate information for the third party that will be receiving the record.
  • Step 4 "Notary Public or Justice of the Peace Acknowledgement" - You must have your signature notarized by a Notary Public/Justice of the Peace.
  • Step 6 "Important!!! Please read the penalty clause below" - The third party requestor must read the penalty clause.
  • Step 7 "Signature is required" - The third party requestor must sign and date the form.
  • Step 8 "Submit your request" - Present the form with a $15 payment to the Concord DMV office, or you may mail the completed form along with the $15 payment to:

Obtaining Another Person's Driver Record Report

You must complete the following fields of the NH MV Record Release FormAdobe Acrobat Reader Symbol(DSMV 505). Please note that these requests may only be processed at the Concord DMV office, as indicated below.

  • Step 1 "What information are you requesting from the DMV?" - Select "Driver record" from the Driver Information heading.
  • Step 2 "Who are you?" - Select the appropriate box provided to indicate under what authority you are eligible to receive the information.
    • Whose information are you looking for? - Fill out the required information on the right with the record holder's information. Minimum information required to complete your request includes:
      • Name (full).
      • Date of Birth.
      • License/Non-Driver Identification Number.
  • Step 3 "Information of the person filling out this form" - Requestors are required to provide their full name, mailing address, and a contact telephone number.
  • Step 4 "Notary Public or Justice of the Peace Acknowledgement" - The named individual in Step 3 must have his/her signature notarized by a Notary Public/Justice of the Peace. If the individual's signature has not been notarized, then Step 5 "Intended Use of Information" must be completed.
  • Step 6 "Important!!! Please read the penalty clause below" - All requestors must read the penalty clause.
  • Step 7 "Signature is required" - All requestors must sign and date the form.
  • Step 8 "Submit your request" - Present the form with a $15 payment to the Concord DMV office, or you may mail the completed form along with the $15 payment to:

Obtaining Another Person's Driver Record - Second and Third Party Requests

If you are acting as an authorized agent of your employer or a company, a Certificate of AuthorityAdobe Acrobat Reader Symbolmust accompany the completed Release of Motor Vehicle RecordsAdobe Acrobat Reader Symbol(DSMV 505) or one must be on file with the DMV. Please note that these requests may only be processed at the Concord DMV office.
