A Letter of Verification is a form that verifies the information of the holder of a NH Driver License or a Non-Driver Identification Card. This information may include the issue date of the Driver License or Non-Driver Identification Card, what restrictions or endorsements the applicant has, their personal identifiable information such as name, address, date of birth and/or driver license number, or any other information that is on file with the DMV.

A Letter of Verification can be used for several different reasons:

  • You have moved to another state and misplaced your New Hampshire license and you need to prove you had a license in NH.
  • Specific information about your license history that is not displayed on the license.
  • Other states requiring specific information about your license at time of transfer.

Letters of Verification may be requested by the applicant or another authorized person by filling out the DSMV 505Adobe Acrobat Reader Symbolform. The cost of a Letter of Verification is $15 and is only available in the Concord office. A request can be made by mail or in person at:

If the applicant is authorizing another person to get this information, either by mail or in person, the applicant must have their signature notarized on the DSMV 505 Adobe Acrobat Reader Symbol, and the authorized person must bring identification, along with the completed form and fee.
