- How do I make a request? What information is needed?
The request must be in writing. The following information is needed:
- Date and type of incident
- Name(s) of subject(s) involved
- Incident location
- Reason for request
- Date, signature and a contact telephone number

- Where do I send the letter?
Please mail the letter to:

- What happens once my letter is received?
After the letter is received, the case will be retrieved and reviewed by the Bureau Commander. If the case is approved for release, the secretary will call you and provide the cost; payment must be received before the report can be mailed.

- What is the fee?
The fee is $3.00 for the first two pages of the report and then $1.00 for each additional page.

- I am an arrestee on a case that has not yet gone to court; may I request a copy of the report from you?
No. The case must be adjudicated before this office is able to send you a copy. You will have to either receive the copy of the report through your attorney, or if an attorney is not representing you, you can request the copy by sending a letter of request for discovery to the prosecution unit associated with the troop station that made the arrest.
