Map & Chart Use

  • Look on the chart for the name of your community. Find the name and address of your reception center in the second column. Write this informaton on the Family Information Form.
  • Find your community on the evacuation route list. Write this informaton on the Family Information Form.
  • Read the suggested evacuation routes. Find the route or routes that you would use to leave the area. Write the route or routes on the Family Information Form.
  • Find the small map that shows the location of your reception center. Put a circle around it. Become familiar with the area. Write the name and address of your reception center on the family information form on the insert page.
  • If your children go to school or day care in a community other than your own, look on the chart for the reception center for their school. (It is the same as the reception center for the community in which the school is located.) Put a circle around it. Write the name and address on your family information form. Find the small map that shows the location of this reception center. Put a circle around it. You would pick your children up there if they had been evacuated.
  • Make sure every member of your family knows where your reception center is.
