Individuals wishing to have pyrotechnic devices reviewed by the New Hampshire Permissible Fireworks Review Committee, are required to submit the following information to the Office of the State Fire Marshal no later than September 1st of the calendar year.
- Written description of the device, including common or trade name.
- Manufacturer name.
- Chemical composition.
- Performance characteristics.
- U.S. Department of Transportation reference (EX) number. Note: This information is usually included in a laboratory report provided by the importer or distributor.
- A product list listing the name, product ID number, and US DOT EX reference number for each item being submitted. The product list shall be written using Microsoft Excel format and shall be submitted to the NH State Fire Marshal’s Office electronically via diskette or e-mail attachment to:
fmo@dos.nh.gov.
- Applications or product lists faxed to the NH State Fire Marshal’s Office WILL NOT BE ACCEPTED.
