7.) The Improper installation of cord connected electric-discharge light fixtures.
Section 410-30 ( c ), (1) permits a listed electric discharge fixture or listed electric discharge fixture assembly to be cord connected provided that the fixture is located directly below the outlet box or busway, the cord is entirely visible outside the fixture and is not subject to strain or physical damage. The outer end of the cord would also have to terminate in a grounded attachment plug cap, busway plug or have a listed fixture assembly with a strain relief and a canopy.
Examples: a fluorescent fixture has been mounted on the surface of a suspended ceiling and cord connected to the outlet box located above the suspended ceiling directly over the fixture. The outer end of the cord has been hard wired directly to the outlet box. There are two problems with this installation. First, the portion of the cord that is above the ceiling tiles is not visible and second, the outer end of the cord has been attached directly to the outlet box. It should have ended in a grounding type cord cap. Another example, probably more common than the first example, is a cord connected HID fixture mounted in an open bay and the cord has been hard wired into an outlet box located directly above the fixture. The outer end of the cord must terminate in a grounded cord cap or a fixture assembly with strain relief and a canopy. In all cases the cord must be entirely visible.
