To obtain a Driving Record, also known as Driver History Record/Report, you must complete the NH Motor Vehicle Record Release Form Adobe Acrobat Reader Symbol (DSMV 505). Note: Driver Record Reports that exceed ten (10) pages in length cannot be printed upon request and will be mailed to the requestor on the next business day. Below, please see the different types of requests and specific instructions to assist you in obtaining a Driving Record:

Obtaining Your Own Driver Record Report

You must complete the following field of the NH MV Record Release Form Adobe Acrobat Reader Symbol (DSMV 505).

  • Box I - Select "Your Motor Vehicle Record"
  • Box II - List your full name, the mailing address to which the record is to be sent, and a telephone number at which you can be reached.
  • Box III - Driver Record select either Certified copy, Non-Certified copy, or Insurance copy.
  • Box V - Minimum information required to complete your request includes:
    • Name (full).
    • Date of Birth.
    • License/Non-Driver Identification Number.
    • Last known address.
  • Box VI - Read and complete the Certification box - sign your full name and date the form. Present it with a $15 payment to the Concord, Manchester, Epping, Dover, or Salem DMV offices, or you may mail the completed form along with the $15 payment to:

Releasing of Your Driver Record Report to Another Individual or Entity

You must complete the following field of the NH MV Record Release Form Adobe Acrobat Reader Symbol (DSMV 505).

  • Box I - Select "Your Motor Vehicle Record"
  • Box II - List the full name of the individual and/or company to whom you want the record to be mailed, the mailing address to where the record is to be sent and a telephone number at which you can be reached.
  • Box III - Driver Record select either Certified copy, Non-Certified copy, or Insurance copy.
  • Box V - Minimum information required to complete your request includes:
    • Name (full).
    • Date of Birth.
    • License/Non-Driver Identification Number.
    • Last known address.
  • Box VI - Present the form in-person to a Notary Public/Justice of the Peace for your signature to be notarized. Read and complete the Certification box, sign, and date the form.
  • Present or mail the completed form along with the $15 payment to:

Obtaining Another Person's Driver Record

You must complete the following field of the NH MV Record Release Form Adobe Acrobat Reader Symbol (DSMV 505).

  • Box I - Select "Another Person's Motor Vehicle Record"
  • Box II - List your full name, the mailing address to where the record is to be sent and a telephone number at which you can be reached.
  • Box III - Driver Record select either Certified copy, Non-Certified copy, or Insurance copy.
  • Box IV - Must be completed if the individual's signature has not been notarized.
  • Box V - Minimum required information on the person whose record you are requesting:
    • Name (full).
    • Date of Birth.
    • License/Non-Driver Identification Number.
    • Last known address.
  • Box VI - The named individual from Box V must have had his/her signature notarized by a Notary Public/Justice of the Peace. If the individual's signature has not been notarized, then Box IV must be completed. You (the requestor) must read and complete Certification box, sign, and date the form.
  • Present or mail the completed form along with the $15 payment to:

Obtaining Another Person's Driver Record - Second and Third Party Requests

If you are acting as an authorized agent of your employer or a company, a Certificate of Authority Adobe Acrobat Reader Symbol must accompany the completed Release of Motor Vehicle Records Adobe Acrobat Reader Symbol (DSMV 505) or one must be on file with the DMV.
