A Letter of Verification is a form that must be requested by an applicant and is not made available without an official request. The official request can be made by filling out the DSMV 505 form and checking the appropriate box. The cost of a Letter of Verification is $15 and is only available in the Concord office. A request can be made by mail if the DSMV 505 is signed by the requestor and the signature is notarized.

A Letter of Verification can be used for several different reasons:

• You have moved to another state and misplaced your New Hampshire license.
• Specific information about your license history that is not displayed on the license.
• Other states requiring specific information about your license at time of transfer.
