   Pharmacist Reciprocity Information

 


The preliminary application for transfer of licensure (reciprocity) should be completed and sent, with the appropriate fee, to NABP (request a reciprocity packet now - be sure to include your full name and mailing address with your request).  Following verification of your credentials by NABP, you will receive an official application with further instructions. After you have finalized the application, submit it to this office along with:

(1) a copy of your birth certificate;

(2) a recent (notarized) photograph;

(3) the $265.00 New Hampshire reciprocity fee in the form of a personal check, money order or bank check - payable to the NH Board of Pharmacy.

A Multistate Pharmacy Jurisprudence Examination (MPJE) Registration Bulletin will also be included in your reciprocity packet. The registration form must be completed (found in the back cover section) and returned to this office along with the $185 examination fee in the form of a money order or bank check (no personal checks) payable to NABP.

In addition, an official finalized transcript SENT DIRECTLY (to this office) from the college of pharmacy that you graduated from must be received at this office in order to complete your application. You are advised to request your transcript as soon as possible in order to avoid delays. A complete copy of the NH Pharmacy Laws and Rules Handbook and the CFR 21, Part 1300 will be sent to you once your MPJE application is received in this office.
