Pharmacy Technicians
All Pharmacy Technician Registrations expire annually on March 31st regardless of date initial registration is issued. There is no statutory provision for extended or pro-rated initial license periods.
Non-Resident / Mail-Order Pharmacies
All Non-Resident / Mail-Order Pharmacy Registrations expire annually on March 31st regardless of date initial registration is issued. There is no statutory provision for extended or pro-rated initial license periods.
Manufacturers / Wholesalers / Distributors / Brokers of Prescription Drugs/Devices at Wholesale
All Prescription Drug/Device Manufacturer/Wholesaler/Distributor/Broker Licenses expire annually on June 30th regardless of date initial license is issued. There is no statutory provision for extended or pro-rated initial license periods.
NH In-State Pharmacies
All In-State Pharmacy Permits expire annually on December 31st regardless of date initial license is issued. There is no statutory provision for extended or pro-rated initial license periods.
Limited Retail Drug Distributors (LRDD's)
There are 3 sub-types of LRDD's - Please select one of the applications below depending on the type of facility you are licensing:
All Limited Retail Drug Distributor Licenses expire annually on June 30th regardless of date initial license is issued. There is no statutory provision for extended or pro-rated initial license periods.
NH Pharmacist Applications
Information / Applications for Initial Pharmacist Pharmacist Licensure in New Hampshire (Not Renewals)
