Credit Card Authorization Form For New Applications

• If you prefer to pay your application/license fees by credit card, download a credit card authorization form to send in with your application. Please note, however, due to credit card security regulations, we are unable to keep your credit card number on file and this form will be destroyed after your initial application fee is processed.
• We accept Visa and MasterCard.

Below is a list of various forms/applications which are available for download in Adobe Acrobat format. Simply download the form to your computer and print it out so that it can be filled out by hand or by typewriter.

Pharmacy Technicians

• Pharmacy Technician Application 
  For New Applicants Only - Do Not Use For Renewals

All Pharmacy Technician Registrations expire annually on March 31st regardless of date initial registration is issued. There is no statutory provision for extended or pro-rated initial license periods.

Non-Resident / Mail-Order Pharmacies

• Non-Resident / Mail-Order Pharmacy Application
  For New Applicants Only - Do Not Use For Renewals

All Non-Resident / Mail-Order Pharmacy Registrations expire annually on March 31st regardless of date initial registration is issued. There is no statutory provision for extended or pro-rated initial license periods.

Manufacturers / Wholesalers / Distributors / Brokers of Prescription Drugs/Devices at Wholesale

• Prescription Drug / Device Manufacturer / Wholesaler / Distributor / Broker - Initial License Application  
  Complete and return this application if you are manufacturing and/or providing these products to NH entities at wholesale only and not to the end-user (e.g. a patient). 

All Prescription Drug/Device Manufacturer/Wholesaler/Distributor/Broker Licenses expire annually on June 30th regardless of date initial license is issued. There is no statutory provision for extended or pro-rated initial license periods.

NH In-State Pharmacies

• Amend-A-Pharmacy Permit Form (NH In-State Pharmacies Only)
  Used to change the pharmacist-in-charge or to change the name of a pharmacy at a NH licensed in-state pharmacy only. (For In-State Pharmacies Only. Non-Resident / Mail-Order Pharmacies May Report Change of Pharmacist-In-Charge Or Pharmacy Name Change On Company / Pharmacy Letterhead).

All In-State Pharmacy Permits expire annually on December 31st regardless of date initial license is issued. There is no statutory provision for extended or pro-rated initial license periods.

Limited Retail Drug Distributors (LRDD's)

There are 3 sub-types of LRDD's - Please select one of the applications below depending on the type of facility you are licensing:

• LRDD - Medical Gases and/or Prescription Devices Direct to Consumer - Initial License Application
  
• LRDD Application - NH In-State Public Health Clinic - Initial License Application
  
• LRDD - NH In-State Methadone Maintenance / Detox Facility - Initial License Application

All Limited Retail Drug Distributor Licenses expire annually on June 30th regardless of date initial license is issued. There is no statutory provision for extended or pro-rated initial license periods.

NH Pharmacist Applications

Information / Applications for Initial Pharmacist Pharmacist Licensure in New Hampshire (Not Renewals)

• Original Licensure in NH (NAPLEX)
• NH License By Score Transfer (Must be Done within 90 days of passing NAPLEX)
• License Transfer - Transfer of a Current Pharmacist License from Another State (a.k.a. Reciprocity)
