New Hampshire Board of Pharmacy License/Registration Verification Page

Requests for all verifications that require more information than can be found below (i.e. full disciplinary history or Board certified verification of licensure), must be submitted in writing (by mail if paying by check - $5.00 per request payable to Treasurer, State of NH) or by fax (603-271-2856) if paying the $5.00 fee per request by credit card (print form out here) - please do not submit the credit card authorization form by email as this is not secure). Only one licensee verification per request (do not submit multiple licensees on one request). Please remember to provide in your request either the email, fax , or postal address of the person/entity to receive the verification, as well as the full name, license type, and NH license # of the person/business you are seeking verification for.
