Credit Card Authorization Form For New Applications

If you prefer to pay your application/license fees by credit card, download a credit card authorization form to send in with your application. Please note, however, due to credit card security regulations, we are unable to keep your credit card number on file and this form will be destroyed after your initial application fee is processed. We accept Visa and MasterCard.

In-State Pharmacy Forms (ONLY FOR USE BY PHARMACIES LOCATED IN NEW HAMPSHIRE)

New Pharmacy Application (NH In-State Pharmacies Only)

All In-State Pharmacy Permits expire annually on December 31st regardless of date initial license is issued. There is no statutory provision for extended or pro-rated initial license periods.

Amend-A-Pharmacy Permit Form

The form below is used to change the pharmacist-in-charge or to change the name of a pharmacy at a NH licensed in-state pharmacy only.

Prescription Drug/Device Manufacturers/Wholesalers/Distributors or Brokers (Third Party Logistics Companies)

Complete and return this application if you are manufacturing and/or providing these products to NH entities at wholesale only and not to the end-user (e.g. a patient).

Limited Retail Drug Distributors

The Board has 3 subcategories of Limited Retail Drug Distributors (LRDD's) and there annual licensing fees are as follows:

Limited Retail Drug Distributor (Public Health Clinic) – $150.

Limited Retail Drug Distributor (Methadone Maintenance / Detox Facility) – $250.

Limited Retail Drug Distributor (Medical Gases and/or Prescription Devices) – $150.
