Pharmacist Licensure Information

Pharmacy Technician Registration Application

All Pharmacy Technician Registrations expire annually on March 31st regardless of date initial registration is issued. There is no statutory provision for extended or pro-rated initial registration periods.

Pharmacy Intern Registration

Non-Resident/Mail-Order Pharmacy Application

All Non-Resident/Mail-Order Pharmacy Registrations now expire annually on December 31st. As the Board is transitioning to this new registration schedule in 2015, the regular annual registration fee of $1,000 has been pro-rated this year as there are only 9-months (3/4 of a year) in this transitionary registration period.

In-State Pharmacy Forms (ONLY FOR USE BY PHARMACIES LOCATED IN NEW HAMPSHIRE)

New Pharmacy Application (NH In-State Pharmacies Only)

All In-State Pharmacy Permits expire annually on December 31st regardless of date initial license is issued. There is no statutory provision for extended or pro-rated initial license periods.

Amend-A-Pharmacy Permit Form

The form below is used to change the pharmacist-in-charge or to change the name of a pharmacy at a NH licensed in-state pharmacy only.

Prescription Drug/Device Manufacturers/Wholesalers/Distributors or Brokers (Third Party Logistics Companies)

Complete and return this application if you are manufacturing and/or providing these products to NH entities at wholesale only and not to the end-user (e.g. a patient).

With the passage of NH Senate Bill 202 (effective 7/1/15) the Board is now able to license 503B outsourcing facilities. All 503B outsourcing facilities wishing to ship bulk compounded products into NH must print out the application below and submit their completed form with the required attachments and fee.

All Prescription Drug/Device Manufacturer/Wholesaler/Distributor/Broker licenses expire annually on June 30th regardless of date initial license is issued. There is no statutory provision for extended or pro-rated initial license periods.

Limited Retail Drug Distributors

The Board has 3 subcategories of Limited Retail Drug Distributors (LRDD's) and there annual licensing fees are as follows:

Limited Retail Drug Distributor (Public Health Clinic) – $150.

Limited Retail Drug Distributor (Methadone Maintenance / Detox Facility) – $250.

Limited Retail Drug Distributor (Medical Gases and/or Prescription Devices) – $150.

All Limited Retail Drug Distributors (LRDD) licenses expire annually on June 30th regardless of date initial license is issued. There is no statutory provision for extended or pro-rated initial license periods.
