Finger print cards and authorization forms will be provided by the NH Board of Nursing with endorsement and examination application forms. The applicant only needs to submit one card and the board will supply 2 cards per applicant in case of the need for a re-take. The cost of the livescan to the NH State Police is $34.25 and inked card is $45.25. If you are applying on-line, please contact the board for fingerprint cards (boardquestions@nursing.state.nh.us) You will need the specific card from the Board of Nursing as they have a code number that allows communication of the results back to the board. Using a different card may result in the board not receiving results and will delay the licensure process.
A list of law enforcement sites will be provided for applicants to call and arrange for fingerprinting. Applicant will be asked to make an appointment at one of the sites listed. Completed fingerprint cards, authorization forms and fees must be sent to the NH Board of Nursing for review. The Board of Nursing will then send your materials to the NH State Police for processing.

Applicants from outside of NH will be sent fingerprint cards and authorization forms. Those applicants are asked to either use law enforcement sites in their respective states or come to NH and use a NH law enforcement site. Please send all fingerprint cards, authorization forms and fees to the Board of Nursing. The board will then send them to the NH State Police for processing.
Please plan early for this process. We have been advised that the results could take as long as 2 months in some cases.
The board continues to work on other options that will provide additional fingerprinting services and sites in the future.
