| License Verification for RNs and LPNs |
| If you are requesting verification of your NH RN or LPN license because you are applying for a license to practice in another state:
As of January 1, 2006, New Hampshire joined the national license verification center known as NURSYS. In order to provide license verification to another jurisdiction, you must activate your license information by contacting NURSYS directly at www.nursys.com to complete the process on line. The fee for activating your license information is $30.00, payable by credit card if you register on line. License information will be accessible by the Board of Nursing to which you are applying within 1 week of receipt of your application and fee by National Council. The verification report will remain active in the NURSYS data base for 90 days, after which it expires, and will no longer be accessible. If you are requesting verification of your NH RN or LPN license for:
- Commission on Graduates of Foreign Nursing Schools
- Visa Screen
- Case Manager Certification
- Medical Board, or
- Other requirements for verification excluding license verification for another jurisdiction.
Send your request along with a check or money order for the amount of $25.00 made out to Treasurer, State of New Hampshire to:
