Requests for Unrestricting/Less Restriction of Formulary Agents should be forwarded to JHC member 30 days before next meeting.

Process for Unrestricting/Less Restriction of Formulary Agents 1. An ARNP or JHC member requestes an agent be unrestricted or less restricted; 2. BOP representative supplies JHC members with current relevant scientific information detailing the agent found at www.Drugs.com and use the healthcare professional section; 3. The agent is placed on JHC agenda and posted on BON website 30 days prior to JHC meeting; 4. A simple majority will determine the formulary status of each agent reviewed. Agents will be designated as unrestricted, use with collaboration, use with consultation or restricted; and 5. Formulary status will be finalized upon approval of minutes at a subsequent JHC within 3 months.
