Instructions for Completion of the Medication Nursing Assistant (MNA) Annual Review/Site Visit Form
1. Check the listing for your MNA program’s Annual Review due date.
2. Go to home page and click on "forms, applications, and publications" to retrieve On-Site Visit Request Form.
3. Download the On-Site Visit Request form, complete the form, and email to: marylou.moreen@nursing.state.nh.us. |

- 45-60 days notice in advance of the MNA Annual Site Review is requested
- Once the completed form is received, you will be contacted via e-mail to schedule a date for your MNA program visit.
