- Please complete the applicable sections of this four page form, and send along with a $20 check or money order made payable to the Treasurer, State of New Hampshire; to the Commission's address below.
- Note that some amendments require that the licensee's wall certificate and pocket ID card must be sent along with the request form and the amendment fee to the Commission.
