• Please complete the applicable sections of this four page form, and send along with a $20 check or money order made payable to the Treasurer, State of New Hampshire; to the Commission's address below when required.
  The $20 amendment fee is only required when the license needs to be amended.

• Note that some amendments require that the licensee's wall certificate and pocket ID card must be sent along with the request form and the amendment fee to the Commission.
