How do I register an alleged misconduct?

Send a letter to the Board with the specific facts and circumstances that you believe to constitute professional misconduct. Please include the name and address of the licensee you are complaining about as well as your name, address and phone number.  You may write your own letter or use the form found under the Consumer section of this website.
