How do I register an alleged misconduct?

Please complete the Report of Alleged Misconduct Form which can be downloaded from the Board's homepage. Click on Applications/ Forms and it will be under General Forms.  If you cannot download the form please feel free to contact the Board office at 271-6762 and ask that a Report of Alleged Misconduct form be sent to you.
