CANDIDATE FOR LICENSURE: SUPERVISION AGREEMENT

If you wish to be licensed in NH and obtain your post-master's hours/post-doc hours in New Hampshire you must have these hours approved by the Board via a Candidate Agreement.   If you do not have an approved Candidate Agreement on file with the Board your supervision hours will not count towards licensure.  This form can be found under Applications and Forms.
