How Do I File a Complaint?

First of all, call 1-800-780-4757 from anywhere in New Hampshire. This toll-free telephone line is available only to consumers for requesting physician information and for help in filing complaints.

By law, all complaints must be received in writing. When you call the 1-800 line, you may ask any questions that will help you decide if you should file a complaint. If you decide to file a complaint, you will be asked for your name and address and a complaint form will be sent to you to assist you in providing all the information that the Board will need to start an investigation.

You will need to provide the following information:

The name of the physician against whom you are complaining. The physician’s address and phone number, if you have them.

The name of the patient, if other than yourself, and your relationship to the patient.

The name of any other physicians or clinics which have been consulted regarding the treatment complained of, and the addresses, if you have them.

The dates of the treatment.

All significant details about the complaint. You may use as many additional sheets as necessary to describe the incident(s) in question. It is best to stick to the facts and details, but your impressions are important too.

You may wish to attach copies of any documents you have which support your complaint. DO NOT SEND ORIGINALS as you may need them later.

You are also asked to sign a "Release of Information" so the Board may obtain all the necessary medical records and billing records.

State law requires that the records be kept confidential and the Board is also required to honor that confidentiality. If you are concerned about the release of medical records, please call the 1-800 number listed on this brochure to discuss your concerns.

In most cases, your complaint will be shared with the physician in order to allow a full response to the allegations. If you are concerned about the physician seeing your complaint, please call the 1-800 number to discuss your concerns. If, after discussing your concerns, you do not wish the information to be shared, please make that wish clear on your complaint. You must understand, however, that such a request may seriously hinder the Board’s investigation into your complaint.
