Complaints may be made by:

- by the web, going to the Board of Medicine’s homepage, clicking on “Contact us” and then emailing or calling us using the information provided.
- also by the web, going to the Board of Medicine’s homepage, clicking on “Consumers” and filling out and mailing the Complaint Form provided.

You will need to give details of what triggered this complaint, including name of the physician, dates of treatment, and all significant details of the treatment or problem. You will also need to provide your name and contact information.
