An affidavit form is needed for any and all officers of a corporation, members,managers of LLC, all partners in a partnership, sole proprietor and any person who is going to be left in charge of the premises at any given time.

CARRIER LICENSE

A carrier license is needed when you are picking up in New Hampshire and taking out of New Hampshire;if you are picking up in New Hampshire and delivering to another part of New Hampshire.

To apply for this license, download the request form, fill out and mail in with a non-refundable $100 processing fee. Once received it will be processed and a license application and checklist of paperwork will be sent to you.

LIQUOR AND WINE VENDOR

If you are the primary source for the liquor and/or wine products you wish to sell to the State of New hampshire then you would need this license.

To apply for this license, download the request form, fill out and mail in with a non-refundable $100 processing fee. Once received it will be processed and a license application and checklist of paperwork will be sent to you.

Liquor and Wine Vendor Request

LIQUOR WINE REPRESENTATIVE

This license is needed in order to represent liquor or wine product(s) for Liquor and Wine Vendors in the State of New Hampshire. In order to obtain this license you must have been a resident of New Hampshire for the past 3 years and be 21 years of age or older.

This license is needed if you have brands of beverages (beer) that you wish to sell to licensed wholesale distributors in this state.
