Liquor and Tobacco License Request for Application

Be sure to complete all four steps.

Step 1. Download and complete the 2 page request for application, all questions must be answered and signed by a person who is authorized by the entity. Print and mail request for application, include non-refundable processing fee, made out to NHLC, Division of Enforcement and Licensing, PO Box 1795, Concord NH 03302.

Step 2. Select the type of entity or non-entity from the list below. Print all documents required for that type of licensing. Once you possess all documents required for licensing (All documents must be in the name of the entity or non-entity that is to be licensed), call the licensing help desk at (603) 271-3523. The Licensing help desk will contact the area Investigator to conduct an inspection of the premise.

Step 3. The day after the inspection is completed, call the Licensing help desk at (603) 271-3523 to schedule a final appointment.

Step 4. Bring copies of all the paperwork that was noted on the checklist to the final appointment. All documents must be in the name of the entity or non-entity that is to be licensed.
