An affidavit form is needed for any and all officers of a corporation, members,managers of LLC, all partners in a partnership, sole proprietor and any person who is going to be left in charge of the premises at any given time.
This form is to be filled out by holders of all private club licenses and must be submitted by the 15th day of each month.
This form is to be filled out by private club licenses who are renting their hall, with or without the sale of alcohol, and must be submitted at least 5 working days prior to first date requested.
Liquor Rule 501.15 Transfers.
(a) All petitions pursuant to RSA 178:25 for transfer of a club license received in the commission's offices at least 5 days prior to the event which meet the requirements of RSA 178:25 and any other statutory requirements which may apply shall be granted.
(b) Transfers shall not exceed 3 consecutive days and 6 total days for any organization during their license year.
The Caterer Site Approval Form is to be filled out by caterers who are looking to have a new temporary or permanent site approved. This form must be submitted with all the appropriate paperwork at least 10 days prior to date requested.
The Property Owner Approval Form is for owner of the property on which the caterer is going to be holding the event(s).
The 5 Day Notice Permanent Site Form is when you have a permanent site already approved and just need to notify us of the event you have scheduled. This form must be in the office no later than 5 days prior to the first scheduled event.
