How to Obtain A Certified Copy of Your Divorce Decree 

If you need to show paperwork for a change of name or to get remarried, you will need a certified copy of the VS (vital statistics) form.  To obtain a certified copy from the court, fill out the Request for Certified Copy of Vital Statistics Form and send the completed form, along with $5, to the court.

If you need a certified copy of the divorce and all the related paperwork, fill out the Request for Certified Copy of Divorce Decree Packet form and send the completed form, along with $15, to the court.

Please Note:  It is recommended that you send a cashier's check or a money order as in some courts your personal  check may be held until it clears from the bank.
