Affidavit of Nursing Home Administrator (NHJB-2147-P 02/01/2008)Agreement (N HJB-2202-DFPS 10/31/2006). Other jurisdictions have been using this form which might be useful to the probate courts.
Department of Health and Human Services Record Release Authorization (NHJB-2171-FP 02/01/2008). Statutory reference (464-A:4, V) was added to heading.
Deposition in Proof of Will (NHJB-2124-P 02/01/2008) . Added statement at the beginning of the form to indicate that the Deposition form cannot be filed prior to the court granting the “Motion to Prove Will by Deposition” (NHJB-2130-P). Question 3 now asks for the date of the instrument in question.
Limited Appearance or Withdrawal By Attorney (NHJB-2294-DFPS 02/26/2008). This form was revised to provide a space for the client's address when the appearance is first filed. The address is required under the court rules.Motion for Summary Administration (NHJB-2149-P 02/26/2008). Added lines to allow for a second executor/administrator.
Motion to Pay State Treasurer For Property in Excess of $5,000 (NHJB-2499-P02/01/2008)
RSA 561:10 allows for an estate administrator to petition the probate court if they have property in excess of $5,000 belonging to an heir, legatee, etc. whose place of residence is unknown and cannot be found, or who may have refused to accept the sum. The petition asks permission to pay the amount to the state treasurer and thus be discharged of their administrator's duties as they relate to the property.
Petition for Administration of Estate Of Person Presumed Dead (NHJB-2497-P 02/01/2008). This petition is an updated version of very old forms 074 and 081. See the Instructions for completing this form for more information.
Petition for Guardian of Incapacitated Person (NHJB-2165-P 02/26/2008). Added space for second petitioners and guardians in questions 1 and 5; changed language slightly in question 13 to make the distinction between a temporary guardianship and a long-term one.
Statement for Payment (NHJB-2154-P 02/01/2008). In question #7, we removed the notation regarding the maximum fee to reflect the recent supreme court rule change.Waiver by Surviving Spouse (NHJB-2498-P 02/01/2008). RSA 560:10 allows a surviving spouse to waive the provisions of a will and also their homestead rights, and instead receive their statutory share of the deceased's estate as defined in this statute. This form is the notification to the court of that action and is an updated version of old form number 137.

Certification of Copies (NHJB-2148-P). The explanation for the case name section has been corrected to apply to all cases.
Petition for Guardian of Minor (NHJB 2162-P). The instructions for question #17 now refer people to the probate court rules for additional information about filing requirements.
Petition/Motion (NHJB-2128-P). The instructions now refer people to the probate court rules for additional information on doing a petition/motion in equity cases.
Some forms had previously been listed in multiple places such as the case-specific list, the “Other Forms” list and the “Numeric” list.
