| You can fill out the juror questionnaire on your computer, print it out, date it, sign it and mail it to court that sent you your jury notice. Or you can complete the copy you received in the mail, date it, sign it and send it to the appropriate court. |

| REMEMBER: Requests for exemptions and postponements must be made in writing and received by the court along with the completed juror questionnaire by the date specified in your letter from the court.

Open the juror questionnaire on your computer and fill in ALL of the fields. Once you have filled out the
questionnaire, you cannot save it to your computer. You must print the document before you close (or exit) the document. Otherwise, your changes will be lost.
Sign and date the printed document mail it back to the court that sent you the summons. |
