The renewal fee is $150.00 for two years.
There is a $30.00 late fee for each month or portion
of a month that your renewal is late.

 

If your license has lapsed for more than 12 months
you must contact the Board office to
file a reinstatement application.

If you do not know when your license expires
you can look it up on the licensee look up
