 The following forms are posted for your use. Please feel free to download or print them. The form must be completed fully, signed, dated and mailed to the Board office with the correct fee.   Incomplete applications will be returned.  

All fees are non-refundable and non-transferable.

Architecture firms must renew or apply by downloading paper forms below.
