The renewal fee is $150.00 for two years.There is a $30.00 late fee for each month or portion of a month that your renewal is late.

 

If your license has lapsed for more than 12 months
you must file a reinstatement application.

The Board considers disciplinary action to be any Settlement Agreement, Consent Order, Board Order or any action by another Board or jurisdiction regarding your license.

Failure to provide accurate answers on your license renewal will result in disciplinary action.
