BASIC PURPOSE: To supervise
and provide direct care services for assigned long-term geriatric residents,
including performing para-professional work such as counseling with residents
and acting as liaison between the hospital and community.
CHARACTERISTIC
DUTIES AND RESPONSIBILITIES:

Acts as a group
leader for geriatric residents involved in mental health treatment programs
conducting programs designed to meet individualized needs.
·
Assesses/evaluates
residents' needs and establishes goals/objectives in order to provide the most
beneficial programming tailored to meet resident needs.
·
Plans, coordinates
and implements groups and activities that correspond to the individual goals
and objectives of the residents.
·
Documents resident
response and recommendations for treatment.
·
Participates in
meetings, presents documentation pertaining to resident's level of
participation and response during group activities.
·
Leads and organizes
activities relating to the maintenance of resident living areas and other
primary resident care needs.
·
Assists treatment
teams, with supplemental training received from discipline advisors, in
achieving treatment objectives at the unit level.
·
Supervises, provides
training and monitors activities of lower level Nursing Assistants during
shifts and as assigned.
·
Participates in and
supervises specialized treatment activities such as behavior modification,
recreation therapy, activity therapy and remotivation programs.

FACTORS:
Skill: Requires skill
in recommending routine changes in standardized operating procedures OR in
retrieving, compiling and reporting data according to established procedures OR
in operating complex machines.
Knowledge: Requires
knowledge of business practices and procedures or technical training in a craft
or trade, including working from detailed instructions, to apply knowledge in a
variety of practical situations.
Impact: Requires
responsibility for contributing to immediate, ongoing agency objectives by
facilitating the direct provision of services to the public or other state
agencies. Errors at this level
result in inaccurate reports or invalid test results and require significant
investment of time and resources to detect.
Supervision: Requires
direct supervision of other employees doing work which is related or similar to
the supervisor, including scheduling work, recommending leave, reviewing work
for accuracy, performance appraisal, or interviewing applicants for position
vacancies.
Working
Conditions: Requires performing regular job
functions in an adverse working environment containing a combination of
disagreeable elements which impact significantly upon the employee's capacity
for completing work assignments.
This level includes work-related accidents or assault.
Physical
Demands: Requires medium to heavy work,
including continuous physical exertion such as frequent bending, lifting or
climbing.
Communication: Requires
explaining facts, interpreting situations, or advising individuals of
alternative or appropriate courses of action. This level also requires interviewing or eliciting
information from state employees or members of the general public.
Complexity: Requires a
combination of job functions using minimal judgment to perform a variety of job
tasks according to clearly prescribed standard practices and procedures.
Independent
Action: Requires making a limited number of
choices in selecting among alternative courses of action under supervisory
guidance and in performing job functions according to a variety of prescribed
policies or procedures.
MINIMUM
QUALIFICATIONS:
Education: Associate's
degree from a recognized college or technical school with major study in the
social sciences or human services; OR a high school diploma plus 18 credit
hours in the field of social services or human services.
Experience: Two years of
experience as a licensed Nursing Assistant. Each additional year of approved work experience may be
substituted for one year of required formal education.
License/Certification: See Special
Requirements listed below:
SPECIAL
REQUIREMENTS:
1.
Must possess current license as a Certified Nursing
Assistant as approved by the New Hampshire Board of Nursing.
2.
Satisfactory completion of in-service hours, within the specified time
frame, as required by HCFA.
RECOMMENDED
WORK TRAITS: Broad knowledge of mental illness and
the social, economic and psychological aspects of illness for the individual
resident. Working knowledge of
hospital and community programs and how to engage them for resident care and
treatment. Skill in basic
interviewing involving understanding of fundamentals of human development and
behavior. Skill in relating well
to residents, families, subordinates and supervisors as a means of achieving
work goals. Ability to maintain a
rehabilitative attitude and relationship with residents. Ability to work with a professional staff
to achieve specific treatment objectives.
Ability to comprehend, act upon and convey oral and written
instructions. Ability to relate
positively and therapeutically to mentally ill persons and identify the
benefits of various restorative therapies. Ability to maintain update and comprehensive medical
reports, ward schedules and activities programming. Must be willing to maintain appearance appropriate to assigned
duties and responsibilities as determined by the agency appointing authority.
