Please find below the list of dentists and dental hygienists who have had their licenses sanctioned. If you wish to obtain a copy of the disciplinary action, send a check made out to "Treasurer, State of New Hampshire" for the amount indicated as "cost". Mail your written request and check to the NH Board of Dental Examiners, 2 Industrial Park Drive, Concord, NH 03301. If you have any further questions, please contact the Board’s office at (603) 271-4561.
This information is provided by the New Hampshire Board of Dental Examiners (Board) for public access to disciplinary actions taken by the Board consistent with the New Hampshire Right-To-Know law RSA Chapter 91-A, and the Board’s enabling statute, the Dental Practice Act, New Hampshire RSA 317-A.
