Occupational Therapists licenses expire on December 31 in the odd-number years.

Every OT and OTA who uses physical agent modalities needs to apply for Board certification in PAMs to use physical agent modalities in the course of their practice with clients or patients. This includes individuals who have been using PAMs under the grandfather clause that was effective on July 1, 2003.

Memorandum from HP and NH Medicaid:

The New Hampshire Occuational Therapy Governiong Board received a Memorandum from HP and NH Medicaid and addressed to All Providers of Physical, Occupational, and Speech Therapy dated June of 2011. The subject is "Clarification on Physical, Occupational, and Speech Therapy Services Provided by Therapy Assistants. The Board feels that it is important for all licensed professionals to have access to this information, a copy of which is attached below:

"All Providers of Physical, Occupational, and Speech Therapy"

Important Licensure Information:

The criminal offender record report requirement includes all States where a student has completed all or part of their clinical experience.

License Renewal

Occupational Therapists licenses expire December 31, 2013. Renewal packets will be mailed no later than November 1, 2013.

If a complete renewal application is not hand delivered or postmarked by December 1st of the renewal year you will be assessed a late fee.

State mail is not forwarded. To receive the packet we must have your current home mailing address on file.
