PROCESSING APPLICATIONS FOR LICENSURE AND ISSUING THE ISSUING LICENSES
The State of New Hampshire is converting to a new licensing data base for all Boards under the Office of Licensed Allied Health Professionals. The license system will be closed from May 2 and will reopen on May 11. APPLICATIONS FOR LICENSURE CANNOT BE PROCESSED OR LICENSES ISSUED DURING THIS PERIOD.

Simply go to the "Forms and Documents" page of this web site click, on the appropriate link, fill in the information, and hit the "Submit My Information" button. It's that easy!
To apply for an initial New Hampshire license or to reinstate a New Hampshire license, go to "Forms and Documents" and use our electronic submission feature to request an application packet or if the feature does not perform properly contact the Office of Licensed Allied Health Professionals at 603-271-8389 and request an application packet.
NOTE: All material to be placed on the Board's agenda must be received no later than noon the business day before the Board meeting in order to go to the Board at their regularly scheduled meeting.
