The New Hampshire Office of Licensed Allied Health Professionals has been established to identify for the public, quality healthcare professionals through a system of licensure, standards of practice, continuing competency, and regulatory oversight.

MOVING:

On December 18, 2013 the Office of Licensed Allied Health Professionals is moving to the Philbrook Building, 121 South Fruit Street, Concord NH 03301. Processing time for requested materials and licensing may be delayed while the Board's staff is busy packing and unpacking. We appreciate your patience during this time of transition.

Our mission is to assure that the services provided by health professionals are effective and of a quality consistent with the standard of care within each profession, and to safeguard the healthcare consumer against harm which may be caused by unqualified, impaired or unlicensed practitioners. We are charged with maintaining the integrity of the following professions:

Athletic Trainers

Genetic Counselors

Occupational Therapists

Physical Therapists

Respiratory Care Practitioners

Speech-Language Pathologists

Recreational Therapists

The Governing Boards qualify and license individuals, establish and enforce law and administrative rules, and provide information so the public can make informed decisions.

Electronic Forms:

For your convenience you can now request an application, change your employer, or change your home address electronically. Simply go to the "Forms and Documents" page of this web site click, on the appropriate link, fill in the information, and hit the "Submit My Information" button. It's that easy!

Initial Licensure:

To apply for an initial New Hampshire license or to reinstate a New Hampshire license, go to "Forms and Documents" and use our electronic submission feature to request an application packet or if the feature does not perform properly contact the Office of Licensed Allied Health Professionals at 603-271-8389 and request an application packet.

NOTE: All material needed to complete an application for initial licensure or reinstatement of license must be received and your file complete no later than noon the business day before the Board meeting in order to go to the Board at their regularly scheduled meeting.
